Your module is not being displayed because it is configured with an incompatible Module Style.
In the settings panel, change the Module Style setting to Host: Classic - 5_Registration_Banner_Only

The Registration fees for 2017, not including the Fundraiser Fee:
You have two options for the 2017 Fundraiser:
1) Pay a $130 fundraiser fee as a part of your registration, and be reimbursed through the fundraising items you sell. If you choose to participate in the fundraiser, you will receive a combination of one box of World's Finest Chocolate, one Gift Check Book, and ten $5 Split-the-Pot Raffle tickets. The box of World's Finest Chocolate will include 60 candy bars that can be sold at $1 each. The Gift Checkbook comes from BoosterShot and can be sold for $30 (or keep the gift checks for yourself) with 30 gift checks totaling approximately $300 in value from local merchants. You can check out the 2016 gift checkbooks at https://goboostershot.com/. The Raffle will take place at the Opening Day Ceromonies.
2) Pay $100 to buy out of the fundraiser, and you will not be required to sell raffle tickets, gift checkbooks, or the chocolate bars.
